1. Course transfer form (http://www.adm.monash.edu.au/service-centre/forms/) by 30 October 2009 to the address below; and
   
   
2. A Monash Supplementary Information Form (http://www.med.monash.edu.au/medicine/mmc/nutrition-dietetics/documents/internal.pdf) by 6 November 2009.

   Monash Course Transfer applicants who are short listed for interview will need to be available from 30 November to 4 December 2009.
   
   Current Monash students are NOT required to apply through VTAC.
