Placement Details

Diabetes is growing into one of the greatest lifestyle diseases to affect the Australian population. The project that can be conducted at YDHS is scope a Diabetes Clinic. This clinic will be multi disciplinary with the GP being a vital member. The project will involve researching other such clinics from within Victoria, elsewhere in Australia and possibly worldwide. Once some examples of clinics have been collected a program applicable to the Yarram and community and YDHS staff will be developed and presented to YDHS management.

Placement Parameters

The student will be coupled with community service health professionals and will have a highly observational role. The students will be free to enquire about any of the services provided by YDHS and every effort will be made to help students have time in areas they prefer.

The Hours of the placement will be:
Community Nursing 8-3
Other community Services 9-4.30

This placement will allow students to understand the provision of health care in a rural community and to understand interdisciplinary communication and care planning. The placement will allow students to understand the large role that community service provides to enable clients to remain living at home for longer.

Selection Criteria

Students will be required to have basic knowledge about the health service which can be attained from the health service website. Students will need to have a enthusiastic attitude toward learning and understanding that not all clients want strangers through their homes.

Ability to research, report write and to present to an audience are essential skills.

Skills in basic observation measurements such as blood pressure measurement are an advantage but are not essential.
