Allied Health Service inc outpatients, mobile home based service across South Gippsland and Bass Coast. Includes all allied health disciplines excluding podiatry.

Health Promotion Activities as outlined in the organisational Health Promotion Plan.

Please note that GSHS also provides acute based services and this has impact on how community based services function.

 

 

Placement Details

  1. Explore and measure local GP referring patterns to GSHS community services, examine the barriers, how can GSHS better promote our service, do GPs understand the scope of practice of GSHS community based staff. (Develop an appreciation of how and why community organisations deliver services)
  • Audit the GSHS health promotion plan against outlined outcomes contained in plan and the current DHS requirements. (Understand and participate in the basic HP process of program development, planning, implementation, and evaluation.)
  1. Conduct and audit of GSHS pre-visit and environmental checklist proformas, for accuracy and relevance of data. Determine what are the barriers to the successful completion of the checklists for O & HS . (Develop an appreciation of how and why community organisations deliver services)
  • Evaluate the consumer satisfaction of the newly created “multi-disciplinary clinic” based at Korumburra due to start 2008.( Understand the “whole person”, and in particular the social and economic context of health and illness.)
  • Demographics of South Gippsland - Choose a GSHS program and capture the relative demographics. (Articulate the relationships between data, information, evidence, knowledge and informed care.)
  • Local Health Services Directory for Clinician use – (Builds on existing GSHS Allied Health project) (Application of a range of knowledge based systems in clinical practice (bibliographic software, decision support systems.)
  • Identifying barriers to access (non physical) by clients for GSHS programs. (Develop knowledge of barriers faced by people in accessing services, their relevance to medicine and individuals health.)
  • Geriatric Evaluation Management (GEM) or Cardiac Rehabilitation or Community Rehabilitation Centre program– enhancing GP involvement and patient outcomes and maximizing referrals. (Articulate the relationships between data, information, evidence, knowledge and informed care.)
  • GP attitudes to Alcohol and Drug and or Dual Diagnosis clients. (Develop knowledge of barriers faced by people in accessing services, their relevance to medicine and individuals health.)
  • GPs understanding and referral patterns to physical activity programs.(Understand the application of a range of HP theories of change that are used in the development of HP interventions and what roles doctors can play in this.)

 

Placement Parameters
At GSHS it is intended that students will experience a range work structures from self directed opportunities through to active participation in team meetings etc.

  • What are the hours of the placement?

Either 08.30 to 5 pm or 9.00 to 5.30 pm. Or by negotiation
