Conference Leave application forms for MBBS students

  1. Students applying for conference leave must complete the appropriate Year level Conference Leave Application Form – links below.
  2. Forms must be accompanied by a Study Management Plan. For Year 5 students this plan must be signed off by the student’s supervisor prior to submission.
  3.   For Years 1 & 2, forms are to be submitted to the Year 1 and 2 Administrator, Building 15 reception at least one week prior to the leave date.  For Years 3 & 5, forms are to be submitted to the Clinical Site Administrator (CSA) at least four weeks prior to the leave date.  For Year 4 forms are to be submitted to the appropriate Year 4 discipline/site administrator at least four weeks prior to the leave date.
